Social Services
A director of social services is on staff to offer residents emotional support and assistance with personal and financial matters. Our director can assist residents with admission paperwork, applying for Medicaid, social adjustment, discharge planning and more.

Meet the Administrator
Ann Scurlock-Moore has served as the Administrator of AHC Crestview for two and a half years, and she is a licensed physical therapy assistant of 27 years. She has a passion for senior care. Ann grew up in West Memphis, Arkansas and was active in the US Army for 4 years. She is an 11-year cancer survivor. She loves to fellowship with people and spend time with her family.
