American Health Communities (AHC) is a division of American Health Partners and has a long tradition of delivering exceptional nursing home care in homey environment. For more than 30 years, our communities have been providing short-term rehabilitation and long-term care to individuals who cannot be cared for at home but want to be near family or friends.
Today, American Health Communities (AHC) operates 29 long-term care and rehabilitation communities in Tennessee and northern Alabama. Being part of the AHC network means local staff can draw upon the resources and expertise of our corporate clinical services team, who oversees and supports all providers.
AHC providers work closely with other American Health Partners’ affiliates to provide a continuum of acute and post-acute care. Through these collaborative partnerships, we expand residents’ access to high-quality health services and ensure easy transitions to care delivered in other settings.
As an added benefit, AHC long-term residents in Tennessee may be eligible to join our affiliated health plan, American Health Advantage of Tennessee. This specialized Medicare Advantage plan offers coordinated care and special benefits for Medicare beneficiaries living in nursing homes.
